What do I need to submit my Health History Form online?
You must have your WUSTL Key and password to submit your Health History Form online. If you do not yet have a WUSTL Key, you should expect it sometime in June.
What is the deadline for submitting my Health History, Immunization, and Meningitis information?
The deadline for submitting your Health History, Immunization, and Meningitis information for Fall '11 is September 30, 2011. The Health History, Immunization, and Meningitis information must be completed before you can register for Spring '11 classes.
Do I need an examination by a doctor, or a doctor's signature for my Health History Form?
No, we do not require a doctor's examination or a doctor's signature.
Is a physical exam required?
No, we do not require a physical exam.
What do I need to do to be considered compliant?
You must complete the entire Health History, Immunization, and Meningitis information. You will find all three in the "Forms" section of the SHS Student Portal.
