Anonymous Report of Sexual Assault/Rape Form
An anonymous reporting mechanism is available for survivors, friends, acquaintances, or family members of survivors to report incidents of sexual harassment, sexual violence, intimate partner violence and stalking. Note: If you are not the survivor, please get permission to provide information before doing so. This report does not initiate a formal complaint, but rather gives the campus information related to safety concerns. Statistics will be reflected in our annual security report.
The submission of this form does not preclude formal reporting options, rights, or resources. By clicking the “submit button”, this form will be submitted securely online and goes directly to Kim Webb, Director of the Office of Sexual Assault and Community Health Services.
