All patients treated by the Habif Health and Wellness Center have rights and responsibilities. They are as follows:
A. To be provided with courteous, considerate care, as well as being treated with respect.
B. To privacy and confidentiality with regard to treatment and medical records.
C. To review your medical records in the company of a professional, but they remain the property of Student Health Services.
D. To be informed of the effectiveness of treatment, and to know of possible risks, side effects or alternate methods of treatment.
E. To have the choice of a clinician and to change clinicians if desired.
F. To know who is treating you.
G. To refuse treatment, or to ask for a second opinion, or an alternative course of treatment, and to be informed of the medical consequences of your actions.
H. To be informed of personal responsibilities involved in seeking medical treatment and maintaining health and well-being after treatment.
I. Inform healthcare provider of any living will, medical power of attorney, or other directive that could affect care.
J. To bring any dissatisfaction to the attention of the attending health professional, the Student Health Advisory Committee or the Associate Director of Student Health Services.
K. Accept personal financial responsibility for all charges that are not covered by insurance companies.
L. Assurance that there is proper transportation and responsible party for patient if required by provider.
A. To present accurate identifying information. This includes information about all aspects of his/her health condition, any medications, including over the counter products and dietary supplements, and any allergies or sensitivities.
B. To present details of illness or complaint in a direct and straightforward manner.
C. To cooperate responsibly with all persons involved in the health care process.
D. To keep appointments on time.
E. To cancel appointments only when absolutely necessary, and far enough in advance so that other patients might utilize that time.
F. To pay for all services rendered according for any charges not paid for by health insurance.
G. To comply with the treatment plan provided by the health professional.
H. To ask for clarification whenever information or instructions are not understood.
I. To provide both positive and negative feedback to the health professional responsibly for care.
J. Be respectful of all health care professionals and staff, as well as other patients.
K. To provide a responsible adult to transport home from the facility and to stay as needed, if required by the provider.
